Honestly, how do they not have randomized controlled trials on all of these supplements? I mean, it’s just been decade after decade after decade after decade, and no one has found a way to do a real study here? Or no one can immunize their methodology from the pathologies that invalidate the results? Really? It’s hard to believe that a problem that is so universal has been dealt with so… impotently

One distinction often blurred in BPH discussions is prostate size vs urinary function. Some supplements may not shrink prostate volume — but neither does Urorec, which I was prescribed — yet it’s routinely given because it improves function. Symptom relief does not require size reduction.
As for pharmaceuticals, here a translation from a German public-service medical programme (WDR) discussing 5-alpha-reductase inhibitors:
“The side effects are partly due to reduced testosterone action: libido decreases, fertility decreases, erectile problems occur, breast growth can occur. What requires particular attention are depression and mood changes, which can extend to suicidal thoughts. Patients and relatives must be vigilant.” From elsewhere, Urorec effects include “ejaculatory changes, dizziness, nasal congestion and diarrhoea; serious adverse effects include facial or airway swelling.”
In my own case, two urologists prescribed Urorec and mentioned TURP surgery, but neither mentioned supplements. I didn’t follow their advice (yet, perhaps never). After those meetings, I researched and my own conservative approach then started with pumpkin seed extract, then beta-sitosterol and nettle root — aimed at symptom control, not prostate shrinkage. Within 48 hours I had marked symptom relief, and within one week I was sleeping through the night. I may yet add saw palmetto. Prostate volume, symptoms and quality of life are different variables. From a patient perspective, hearing about all options matters. Because supplements are not standardised doesn’t mean good ones are not effective. As background, I’m 70, otherwise fit and lean, i don’t drink alcohol, I eat plant-based. This life style aids reduction of inflammation. I will still monitor prostate size by echo sound or MRI at a medical practitioner.
